• Written, 30-day notice to cancel. 
• Memberships with a 30-day past due will be canceled. However, the Beverly Hills Maintenance Membership can only be canceled after 6 months. 
• To reinstate, all past dues must be collected first.
• Valid credit card and photo ID due upon signing – First and last month dues owed upon signing.
• Any unused monthly services cannot be carried over and hold no cash value. 
• Any unused services included with the membership hold no cash value and can not be paid out upon termination of the agreement or combined with any other refund.
